US Family Health Plan members may receive vaccinations for both the seasonal flu and the H1N1 flu.

Members should first contact their primary care provider's office about vaccine availability. If your PCP does not have the vaccine available, you may obtain it from any health center or organization that does. If you are charged for the administration of the flu vaccine, US Family Health Plan will reimburse you up to $30 .

New Hampshire
Rhode Island
Connecticut

Veterans with proof of military service (Form DD-214 or Military Retiree ID card) are also encouraged to contact their local Veterans Administraction (VA) facility to check for flu vaccine availability.
