Frequently Asked Questions

US Family Health Plan is available to the following beneficiaries living in Massachusetts, Rhode Island, or parts of southern New Hampshire and northern Connecticut: 

  • Active duty family members
  • Retired service members and their family members
  • Family members of activated National Guard and Reserve members
  • Non-activated National Guard and Reserve members and their family members who qualify for care under the Transitional Assistance Management Program
  • Retired National Guard and Reserve members aged 60 and older and their family members
  • Survivors
  • Medal of Honor recipients and their family members
  • Qualified former spouses

The easiest way to enroll is to let us fill out the enrollment form for you. Call us at 1.888.815.5510 or contact your area's representative, and we can complete and submit an enrollment form for you. 

Other ways to enroll include the following:

  • Download and print the enrollment form and fill it out yourself. Mail your completed form to us at US Family Health Plan, PO Box 495, Canton, MA, 02021-0495
  • Enroll online by visiting the milConnect (osd.mil) Beneficiary Web Enrollment (BWE) site. Please note that you can't select a primary care provider (PCP) on the BWE site, so if you enroll online, US Family Health Plan will assign a PCP. If you want to select a PCP yourself, contact your US Family Health Plan area representative and enroll by phone. 

Active duty family members and transitional survivors pay no enrollment fee, no deductible, and no co-pays (except for some prescriptions). Everyone else pays a modest annual enrollment fee.

To find out if your primary care provider is in the USFHP network:

  • Click on Find a Doctor. A new tab or window will open. 
  • Enter your location, either a Zip code or the city/town in which you want to search. 
  • On the dashboard, you can use "Doctors by name" if you know the provider's name and want to see if they are in network.
