Are you eligible?

US Family Health Plan (USFHP) of Southern New England is a TRICARE Prime option available to families of Active Duty service members and military retirees under 65 and their families.

Families of Active Duty service members

Dependents of Active Duty service members, including dependents of Guard and Reserve Component members activated for more than 30 days
Non-activated National Guard/Reserve members and their families who qualify under the Transitional Assistance Management Program (TAMP)
Former spouses of military sponsors who meet TRICARE eligibility rules

Military Retirees under 65 and their families

Military retirees under 65 from the Active component and their dependent family members
Military Retirees ages 60–64 from the Reserve component and their dependent family members
Family member survivors of a Military Retiree who are under 65

Additional eligibility requirements

Be eligible for military healthcare benefits in the Defense Enrollment Eligibility Reporting System (DEERS)
Live in the ZIP Code-defined area that we serve in Southern New England, which includes Massachusetts, Rhode Island, Connecticut, and Southern New Hampshire

Please note: Although their families are eligible, Active Duty service members themselves are not eligible for USFHP coverage, as they receive healthcare from military hospitals or clinics.
