How Does the Plan Work?
Your Primary Care Provider is the hub of your medical care. Our Member Services department helps you, too.
When you enroll in US Family Health Plan of Southern New England, you choose a Primary Care Provider, your “PCP,” from our network of civilian providers. You visit your PCP for annual physicals and when you're sick or injured. If you need to see a specialist, your PCP makes the referral for you. If you need to be hospitalized, receive urgent care, or have lab work or other medical services, your PCP makes the arrangement for you.
You receive most prescription medications through our special Home Delivery mail-order pharmacy. For medications that you will take for only a short time, like antibiotics or pain medication for an acute illness, you go to a retail pharmacy.
